Open the settings by clicking on the gear icon at the bottom left corner.
Navigate to the Licenses section.
Paste the license key into the Bootstrap License field and click Activate.

The bootstrap design is visible on all the different types of fields like radio, checkboxes, inputs, select and textarea. However, if it is not getting applied on any of your form fields then use the contact form and I will add compatibility for it.

Does this plugin use Bootstrap 3 or 4?
The plugin is using Bootstrap 4 as it is the most recent major version of Bootstrap. The library is regularly updated to add the latest available version.

Is it also going to affect other forms or elements of a website?
The bootstrap library is prefixed in such a way that it will only get implemented on WPForms. It won’t affect the CSS of any of your forms on which you haven’t enabled bootstrap.
